How to Register:

Begin by downloading our Parish Registration form:

Option 1:

Physical Registration

  1. Print out the PDF Form

  2. Fill out the PDF document and return it to our
    Parish Office during business hours.

Option 2:

Digital Registration

  1. Fill out the PDF digitally or scan a completed physical copy

  2. E-mail the completed document
    to info@iccmahwah.org
